SI joint pain after pregnancy
Lower back and pelvic pain are common during and after pregnancy. For many women, symptoms settle within a few weeks of giving birth. However, some continue to experience persistent discomfort, often linked to the sacroiliac (SI) joints.
The sacroiliac joints sit at the base of the spine, connecting the sacrum to the pelvis. They play an important role in transferring weight between the upper body and the legs, while also providing stability during movement.

Why does SI joint pain occur after pregnancy?
Research suggests that between 26% and 66% of postpartum women experience sacroiliac joint dysfunction (SIJD) and ongoing pain.
Several factors contribute to SI joint pain in the postpartum period. During pregnancy, the body releases hormones such as relaxin, which increase ligament flexibility to prepare for childbirth. While this is necessary, it can also reduce stability around the pelvis and SI joints.
As ligaments become laxer, the SI joints may move slightly more than usual. This increased movement can lead to irritation, inflammation, and pain. In addition, the growing uterus shifts the body’s centre of gravity, placing extra strain on the lower back and pelvis.
Following delivery, these changes do not immediately reverse. Ligaments may take time to regain their normal tension, and muscle strength around the core and pelvis is often reduced. This combination can leave the SI joints vulnerable to ongoing strain.
Daily activities such as lifting a baby, feeding, carrying, and prolonged standing or sitting can further aggravate symptoms, particularly if the joints remain unstable.
When symptoms persist
SI joint pain is typically felt in the lower back, buttock, or pelvic region. It may affect one side or both, and can sometimes radiate into the thigh. Unlike sciatica, it usually doesn’t travel all the way down the leg to the foot.
Many cases of postpartum SI joint pain improve with rest, massage, physical therapy and rehabilitation exercise. Physiotherapy plays a central role, focusing on restoring strength and stability in the core, pelvic floor, and surrounding muscles.
If SI joint pain continues despite appropriate rehabilitation, further evaluation may be needed to confirm the diagnosis and explore additional treatment options.
