Why is my sciatic pain worse in the morning?

Sciatica is a common cause of leg pain that originates from irritation or compression of the sciatic nerve. This nerve runs from the lower back down through the hips and into each leg, and when it becomes irritated, it can cause pain, tingling, numbness, or weakness.

Many people with sciatica notice their symptoms feel worse first thing in the morning. This can be frustrating, particularly when pain makes it difficult to get out of bed and start the day. Here, we explain why this happens and suggest some simple changes that may reduce discomfort.

Why does sciatica feel worse in the morning?

Several factors contribute to increased sciatic pain on waking. During sleep, the spine remains in a relatively fixed position for several hours. Intervertebral discs naturally rehydrate overnight, absorbing fluid and becoming slightly larger. While this is a normal process, it can increase pressure within the discs, particularly if a disc bulge or herniation is already present.

This temporary increase in disc volume may place additional pressure on nearby nerves, including the sciatic nerve, making symptoms more noticeable in the morning.

Muscle stiffness also plays a role. After a period of inactivity, the muscles and joints surrounding the spine can feel tight and less flexible. This stiffness may exaggerate nerve irritation when first moving, leading to a sharp or aching sensation in the lower back or leg.

Sleep position is another important factor. Certain positions can place the spine in a slightly twisted or unsupported alignment, increasing strain on the lower back and contributing to symptoms.

The role of sleep position

Research into sleep posture and spinal symptoms is still developing, but there are some useful observations. A scoping review found that those who spent more time in symmetrical side-lying positions reported fewer morning spinal symptoms than those in asymmetrical positions.

Symmetrical side-lying generally means lying on one side with the spine aligned and supported, rather than twisted.

Practical adjustments may help reduce overnight strain on the spine:

  • Lying on your side with a pillow between the knees to keep the hips aligned
  • Sleeping on your back with a pillow under the knees to support the natural curve of the lower back
  • Avoiding positions that involve twisting the spine or uneven support

A supportive mattress and pillow can also help maintain a neutral spinal position throughout the night.

Gentle morning movement

How you move in the first few minutes after waking can influence how quickly symptoms settle. Sudden bending or twisting may aggravate nerve irritation, particularly when discs are still under increased pressure.

Instead, gradual and controlled movement is often more comfortable such as rolling onto your side before sitting up, or sitting at the edge of the bed before standing.

Gentle stretching and low-impact exercise can support longer-term symptom improvement. A Pilates-based exercise programme can be effective in improving lower back pain and is generally safe for most patients.

These exercises focus on core strength, flexibility, and controlled movement, which may help reduce strain on the spine and improve support for affected structures.
