Running is one of the most accessible forms of exercise, widely valued for its cardiovascular benefits, weight management, and overall wellbeing. With longer days and lighter evenings, many recreational runners are increasing their training volume, making it a good time to understand how running affects the spine.

The role of spinal discs when running

The spine contains intervertebral discs positioned between each vertebra. These act as shock absorbers, distributing forces generated during movement and physical activity.

Every footstrike during running sends an impact force up through the legs, pelvis, and spine. The discs manage these loads by absorbing pressure and allowing controlled movement between vertebrae. For most people, these forces are well tolerated – and the evidence is broadly reassuring.

Training load and lower back pain

A systematic review of lower back pain in runners reported prevalence rates of just 0.7–20.2%, concluding that running may actually serve as a protective factor against the onset of lower back pain when compared with the general population.

That said, risk factors do exist – and many are modifiable. Sudden increases in mileage, intensity, hill work, or race preparation can place greater demands on the spine before supporting muscles have fully adapted. Almost 80% of running-related injuries are due to overuse, resulting from an imbalance between tissue capacity and biomechanical load – the lower back is no exception.

When running triggers sciatica

Sciatica occurs when the sciatic nerve becomes irritated or compressed, typically causing pain that radiates from the lower back into the buttock and leg, along with tingling, numbness, or weakness.

While sciatica can be debilitating when it does occur, it is not a particularly common running injury. A study examining 800 marathon runners found the incidence of lower back pain was 4.5%, concluding it was uncommon in this group but linked to modifiable factors including insufficient warm-up, fatigue, and poor running gait.

When running does aggravate sciatica, it is usually because repetitive loading irritates an existing disc problem, causing inflammation or pressure on a nearby nerve root. Symptoms typically worsen during or after running, particularly where an underlying disc bulge or degenerative change is already present.

Most running-related back pain responds well to conservative management: a temporary reduction in training load, physiotherapy, and targeted strengthening exercises are often sufficient.
