Lower back pain is one of the most common musculoskeletal complaints worldwide. At the same time, obesity rates continue to rise globally. Increasingly, research shows that these two health concerns are often linked.

A recent large-scale study examining body weight and spinal health found a significant association between obesity and lower back pain. People with higher body mass index (BMI) were more likely to report persistent or recurrent back pain compared to those in a healthy weight range.

While this doesn’t mean that weight is the sole cause of back pain, it does show it’s an important contributing factor. Understanding how excess weight affects the spine can help guide both prevention and treatment.

How excess weight affects the lower back

The lumbar spine is responsible for supporting much of the body’s weight. When body weight increases, so does the mechanical load placed on the intervertebral discs, facet joints, ligaments, and muscles of the lower back.

Over time, this additional strain can accelerate wear and tear. Increased abdominal weight may also alter posture, shifting the centre of gravity forward and placing further stress on the lumbar spine. In addition to mechanical factors, obesity is associated with low-grade systemic inflammation, which may contribute to pain sensitivity and joint degeneration.

Herniated discs and obesity

Intervertebral discs act as shock absorbers between the vertebrae. With repetitive strain and increased load, discs may degenerate more quickly. Research has suggested that obesity is associated with a higher risk of lumbar disc herniation.

A herniated disc occurs when the soft inner portion of the disc protrudes through its tougher outer layer, potentially compressing nearby nerves. This can result in lower back pain, sciatica, leg numbness, or weakness.

Although disc problems can occur in those of any weight, managing overall spinal load is an important component of reducing recurrence risk.

Can weight loss improve lower back pain?

Encouragingly, research suggests that structured weight loss interventions may positively influence lower back pain outcomes. A systematic review examining weight loss programmes for those with back pain found that participants who achieved weight reduction often reported improvements in pain levels and physical function.

Weight loss may reduce mechanical stress on spinal structures and improve mobility, making rehabilitation exercises easier to perform. Even modest weight reduction can lower compressive forces on the lumbar spine.
